En el ámbito de la salud pública cuando una persona concurre al encuentro de un profesional, suele ser recibido y atendido en una primera escucha que desencadena un interesante entramado de fichas, historias clínicas y urgencias que implica ubicar al potencial paciente en algún dispositivo; al cual ingresará de distintas formas pero con una condición sine qua non: el diagnóstico presuntivo según la formulación del DSM o el CIE-10, ambos manuales diagnósticos y estadísticos de los trastornos mentales. En nuestro trabajo, a partir de un paciente internado en una Unidad de Desintoxicación por su consumo de Pasta Base de Cocaína, intentaremos dar cuenta de la dificultad que plantea asociar indefectiblemente el consumo de un tóxico con la estructuración subjetiva de un sujeto adulto, y las diferencias entre la escucha psicoanalítica -allí donde un analista se constituya- y el diagnóstico psiquiátrico, requiriendo calificación y clasificación de un consultante. In the ambience of the public health when a person meets to the meeting of a professional, it usually is received and attended in the first listening that unleashes an interesting framework of cards, case histories and urgencies that it implies locating to the patient potential in some device; to which it will enter of different forms but with a condition sine qua non: the diagnosis presuntivo according to the formulation of the DSM or the CIE-10, both diagnostic and statistical manuals of the mental disorders. In our work, from an inpatient in a Unit of Detoxification for his consumption of Basic Pasta of Cocaine, we will try to realize of the difficulty that raises to associate unfailingly the consumption of a toxic with the subjective structure of an adult subject, and the differences between the psychoanalytic listening - there where an analyst is constituted and the psychiatric diagnosis, needing qualification and classification of a consultant.
